The courage of these individuals would have never been necessary, and the tragic loss of life never would have happened, if government policies hadn't failed in the first place. The Coroner's report makes clear that policy decisions left a dangerous individual unsupported and unsupervised in the community, with ordinary people paying the price for this negligence.
